On Tuesday, Oct. 11, at 1 p.m., at the Fairview Chisago Lakes Health Center in Chisago City, Fairview Lakes Social Worker Lisa Graham, MN, L.S.W., will guide a discussion to help you talk about these important issues and when to complete a health care directive. Her talk is titled,ÝìCourageous Conversations: Making your Choices about Life and Death Known.îÝ

Health care directive forms will be available for those who want them. Pre-registration is not required.

This free program is part of the ongoing ìTodayís Health Topicsî series sponsored by Fairview Lakes Health Services and the Chisago Lakes, North Branch and Forest Lake community education programs. All interested persons are welcome to attend. Refreshments will be provided.
